Safety Assessment Scale
A screening tool for the person with dementia living at home. A score of 11 to 14 indicates a moderate risk of accident. A score of 15 or more indicates a high risk of accident. The long version should be used to perform an in-depth evaluation when the result on the short version indicates a moderate or high risk of accident.
